About Us

Transition Housing are a non-profit organisation providing supportive housing to vulnerable adults from all communities and backgrounds with an aim to help individuals make a positive transition in life.

Once we have assessed an individual and deem them to fit the necessary criteria we can offer accommodation regardless of gender or risk level.

A wide range of support is delivered internally and externally in order to aid with the building and sharpening of essential skills necessary to become an independent member of the community. Once any client acquires independency through our move on scheme and becomes a former client Transition Housing will continue to offer support to that client for a period of six weeks ensuring our clients are confident in the transitional process and has every chance of success with independency.

Our properties are all located in residential areas, close to local amenities, leisure facilities and employment opportunities. We provide a safe and homely environment, where personalised support is delivered to help people live as independently as possible.

Through elements of communal living, residents make new friendships, develop social skills and can get involved in one on one or group sessions such as cooking, employment work shops and other various tailored in house workshops.
